Deerfield Charitable Foundation Funds Care Navigator at MemoryCare
MemoryCare is proud to announce a $70,000 grant from the Deerfield Charitable Foundation to expand the reach and impact of its Guiding an Improved Dementia Experience (GUIDE) program.
The GUIDE model, a specialized Medicare-supported initiative, is designed to enhance care coordination, reduce caregiver burnout, and improve the quality of life for individuals living with dementia. While the need for these services across Western North Carolina continues to grow, maintaining the details of monthly outreach and monitoring activities required by the program demands significant staff resources.
Thanks to Deerfield’s generous investment, MemoryCare is adding a full-time Care Navigator to the GUIDE team.
This new role will provide the extra support needed to:
-Provide regular check-ins and monthly outreach with enrolled patients and their families
-Make home visits to provide hands-on navigation and help older adults safely age in place
-Offer caregivers community resources, education, and respite options
-Expand program capacity to move MemoryCare toward its target census of 200 participants, ensuring eligible local families receive timely care
By funding this role, the Deerfield Charitable Foundation is not only helping more families access life-changing dementia care, but also securing the stability needed to sustain the program for the future.
MemoryCare sends its heartfelt appreciation to the Deerfield Charitable Foundation for their continued commitment to the well-being of older adults and caregivers throughout our community.
