MemoryCare’s strength lies in the personal attention families receive during and between appointments.

In order for us to provide the most comprehensive and beneficial treatment plan for your loved one, your first appointment will be 3 hours in length, and subsequent appointments will last one hour on average.

In addition to the care provided during scheduled appointments, your care team can provide support between appointments over the phone and via e-mail if permission is given with disclaimer. You will also be connected to important resources, including educational events, support groups in the area, literature available in the Caregiver Library, and web resources. Forms

Referral Form

Because our practice is consultative in nature, we require a referral in order to schedule your initial appointment. We prefer that this referral come from the patient’s primary care physician or neurologist.

(Please note, this must be filled out by a health care provider.)

New Patient Pre-Visit Forms

If you have spoken with our scheduler and prefer to expedite your paperwork instead of waiting for your packet to arrive in the mail, please complete the pre-visit forms and return to MemoryCare along with your non-refundable deposit check for $100, or you can pay by credit card over the phone when the scheduler calls to make the appointment.
